Tips on prep

  • Locate your email confirmation for the visit – and download and access the app
  • Choose a quiet place and use headphones so you will be able to hear clearly
  • Make sure you are using a good internet connection (use Wi-Fi or a wired connection for best results)
  • Log on 10-15 minutes before your appointment time

To schedule an appointment, please use our book an appointment on our website under the Telehealth link.

Once you are scheduled for a visit by one of our providers, working together with you, your goals will be identified, a treatment plan will be formulated. With your consent an appropriate referral will be provided upon the provider’s discretion to meet any other specialty health need you may be concerned about.

We strongly encourage at least a 24-hour notice for appointment cancellations or to reschedule to avoid a NO CALL NO SHOW FEE of $50 for new patient appointment (s) and $25 for follow up appointments. To cancel or to reschedule an appointment, please call the office at +1 (877) 885-4088.

If you are in a life-threatening situation or an emergency, call 911 or go to your nearest Emergency room.

During your first visit, you will be required to complete your registration forms, consent forms, intake questionnaire. Please kindly complete all the required documents prior to your scheduled appointment. This will enable you to be seen by one of our providers at your scheduled time without much delay.

Once you and your provider have completed your initial appointment, a follow-up appointment will be schedule to continue monitoring your response to treatment plan formulated. During your follow-up appointment, you discuss your concerns with your treatment plan, response and new concerns.

We strongly encourage you to keep your appointment as scheduled as medications are only refilled until your next scheduled appointment. We do understand that emergencies do happen, but if you miss your appointment, please call the office as we can provide you with a 7 days refill until your next scheduled appointment.
